Revive Health & Wellness Stuart, LLC, a Florida limited liability company (document number L19000043266), principal address 838 SW Federal Hwy, Stuart, FL 34994, which operates the Stuart club.
Both companies use the mailing address 2300 SW Gateway Place, Suite 150, Stuart, FL 34997. In this policy "we", "us" and "our" refer to the company that operates the club you contact.
Information you give us. When you complete the form on this site we collect your first name, last name, email address, phone number and the club you selected. We also record whether you consented to receive text messages, and the date and time you gave it.
Information collected automatically. When you visit the site we and our providers may collect your IP address, device and browser type, the pages you view, how you arrived at the site, and similar technical data. This is collected through cookies, pixel tags and web beacons, described below.
We do not ask for payment card details, government identification or health information through this website. Memberships are set up in person at the club.
We use what you give us to contact you about a membership, to book a tour or a first session, to answer your questions, and to send you offers if you asked to receive them. We use technical data to keep the site working, to understand which pages people use, and to measure and improve our advertising.
If you tick the consent box on the form, you agree that we may contact you by text at the number you gave us, about your visit and your membership. Message frequency varies. Message and data rates may apply. Reply STOP to stop receiving messages, and HELP for help. Consent to receive texts is not a condition of buying anything from us, and you can withdraw it at any time by replying STOP or by writing to us at the address for your club below. We do not share your phone number, or the fact that you consented, with third parties for their own marketing.
We use cookies and similar technologies to keep the site working, to remember your choices, and to measure our advertising. These include:
Google Analytics, to understand how the site is used. Google Ads, to measure our search and display campaigns. Meta pixel, to measure our Facebook and Instagram campaigns and to show ads to people likely to be interested in the clubs.
These technologies allow those platforms to receive information about your visit. Under some state privacy laws this counts as "sharing" your information for targeted advertising, even though we receive no money for it. We do not sell your personal information for money.
You can block or delete cookies in your browser settings, and you can opt out of personalised advertising through the settings offered by Google and Meta. Blocking cookies may stop parts of the site from working.
We share information with the companies that run our systems for us, and only so they can provide that service to us. These include our customer relationship platform, which stores your details and our messages with you, and our email and text messaging providers.
We share information when the law requires it, when we need to establish or defend a legal claim, and, if the business is sold or reorganised, with the party that takes it over.
We keep personal information for as long as it is needed for the purpose we collected it, and for as long as your membership is active. We keep it longer where we need to in order to meet a legal or tax obligation, resolve a dispute, or collect amounts owed to us. When information is no longer needed we delete it or make it anonymous. Anonymous and aggregated data, which cannot identify you, may be kept without a time limit.
Depending on where you live, you may have the right to ask what personal information we hold about you, to receive a copy of it, to have it corrected, to have it deleted, and to opt out of targeted advertising. You may also ask someone to make a request on your behalf.
To make a request, write to us at the address for your club below. We will need to confirm who you are before we act. We will not treat you differently for exercising these rights. If we refuse a request we will tell you why, and you may appeal by replying to our decision.
This website is not intended for children. Please do not use it if you are under 13. We do not knowingly collect information from children under 13, and if we learn that we have, we delete it. If you believe a child has given us information, write to us and we will remove it.
You must be 18 or over to submit the form on this site. Anyone under 18 who wants to train with us must be enrolled at the club by a parent or guardian, who signs on their behalf.
We use reasonable measures to protect the information we hold. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
This policy is governed by the laws of the State of Florida.
We may update this policy. When we do, we will change the date at the top. If the change is significant we will say so on the site.
